Sadly, I am not sure of the manufacturer; it was purchased on Amazon and the packaging says — nothing. It is surprisingly heavy, which is not a bad thing.

It is a tripod head, which means it is meant to be mounted on top of a photography tripod. However, if your camera isn’t too heavy, you can simply screw it into the mounting plate and use it on a tabletop or desktop.

To use the tripod head, you need a camera that has a screw mount in the bottom, a tripod, and that’s about it. The head comes with an Allen wrench to adjust the angle of tilt, from completely flat and parallel to the ground to extended and at right-angles to the ground.
